12/23/1992 FABOH Incorporation Papers Filed with 10 Stockholders Initial Stockholders included: Badger Liquor, JF Ahern, Wisconsin Employers Association, Galloway West Company, Wells Manufacturing, Thresherman’s Mutual Insurance (Now Society Insurance), Giddings and Lewis, Mercury Marine, Action Advertiser, and Mid-States Aluminum
7/22/1993 – First Physician Contract Signed

Fall of 2006 – FABOH officially starts the wellness program by bringing on a dedicated wellness professional to develop and oversee the wellness strategy
2010’s
May 2010 – FABOH brings on its first Wellness Intern. The Internship program ran for a number of years, providing members with additional support for employee wellness efforts and afforded interns with a fantastic learning opportunity
4/1/2011 Ripon Medical Center added to the list of Agnesian Health Care Hospitals after it was acquired
4/11/2011 – Larry Richardson named FABOH Executive Director
1/10/2013 – FABOH launches the first all member wellness initiative – Self-care. This initiative had a 97.5% satisfaction rate and led to employer health care cost avoidance of over $100,000 in first three years of existence
7/1/2014 FABOH Advantage added to FABOH’s product lineup. This product combined the strengths of FABOH and Health Payment Systems (HPS) to benefit current and prospective members
1/1/2015 FABOH contracts with Agnesian Health Care creating access to the Corporate Care Clinic, a low-cost primary care option available to all FABOH members
12/15/2016 – FABOH leads the City of FDL and surrounding areas to a Well-City designation through the Wellness Council of America (WELCOA) Through this process, 26 Fond du Lac area employers achieved a Well Workplace designation
2020’s
July 2020 – Jeff Butz named FABOH Executive Director
11/1/2021 – First level funded product member signed – FABOH’s level funded product, created in collaboration with WPS, provides Fond du Lac area employers who aren’t ready to go self-insured to access the FABOH network
12/23/2022 – FABOH Celebrates 30 years of supporting Fond du Lac area employers with maximizing the value of their health care dollar
4/1/2023 – FABOH began offering NICE, an affordable virtual health care tool that small employers can offer their employees to level the playing field when recruiting against employers who are offering a health plan
