Appointments:

You may make an appointment by calling our office at 920-926-5578 during office hours. When calling to schedule an appointment, remember to say you are a Corporate Care Clinic client. We ask that an adult or legal guardian accompany a minor (patient under the age of 18). For the safety of your child, children from birth to five years should seek care with a pediatrician. If you need to cancel or reschedule an appointment, please notify us as far in advance as possible so that your time may be given to another patient in need of care.
